a. Web Server Logs.

e. When you may be contacted by RemakeHealth
RemakeHealth will only contact users under the following conditions:

• To schedule, confirm appointment status or review other information relevant to a specific purchase made through the RemakeHealth website

f. Referral services
If you choose to use our referral service to “email a friend” about our site, we will ask you for your friend’s name and email address. We will automatically send your friend a one-time email inviting him or her to visit the site. RemakeHealth may store this information for the sole purpose of sending this one-time email and/or tracking the success of our referral program.
Your friend may contact us at support@remakehealth.com to request that we remove this information from our database.
g. Imaging Networks
RemakeHealth may contract with 3rd party credit card processing, billing, phone, email, and imaging network companies in order to provide services on our behalf. When you make a purchase on the RemakeHealth website, we may share your contact information such as name, address, telephone number and/or email address as necessary for the third party to provide that service.
These third parties are expressly prohibited from using your personally identifiable information for promotional purposes.
